Your national digital identity (the login you use for tax, benefits, health) is the master key to everything, so treat misuse as an emergency. Then report to Queixa Eletrónica — PSP / GNR (queixaselectronicas.mai.gov.pt); the report number is needed to undo anything done in your name (a changed address, a benefit claim, a filed tax return).
