What to do first: First complain to your operator in writing. It must reply within 15 days. Only if it refuses, does not answer, or you reject its solution can you apply to the committee; attach your complaint and the operator's reply.
Time limit: The Consumer Protection Act's grounds for refusing an application contain no filing deadline. TTJA's complaint guide says to complain to the trader without delay, at the latest two months after finding a defect; for a service, within two years of receiving it.
